Referral from the Occupational Therapy Service

In order for Ashfield Homes to make any adaptations to your home, a referral will normally be required from the Nottinghamshire County Council Occupational Therapy Service. However, some small adaptations can be completed following a referral carried out by our own employees. Upon receipt of this referral, we will make, whenever possible, the specified alterations to your home.  Should you consider that any adaptations are required to your home, please contact the Repairs Call Handling Centre on 0800 479 4999 (Freephone) for further assistance or complete the on-line form at the foot of this page.
