The support plan

 

Based on the criteria from your assessment, the Tenancy Support Officer will put together a Support Plan. The Support Plan is designed to give a responsive service to meet your needs and help you to sustain your tenancy. Your Support Plan can be adapted at any time should your circumstances change, and we will review your Support Plan on a regular basis to ensure your needs are met.

 

The Support Plan will help you to:

  • Understand your rights and responsibilities as a tenant.
  • Assist you with budgeting, debt counseling, and welfare rights.
  • Assist you to apply for welfare grants or loans and obtain furniture and necessary household equipment.
  • Advise you of appropriate local groups and activities.
  • Help you register an account with Gas and Electric services.
  • Help with the completion of Housing Benefit and Council Tax Benefit forms.
  • Assist you to register a Severn Trent Water account.
  • Help you to obtain a TV Licence.
  • Help to set up the Home Visiting or Call Monitoring Service should this be required.
  • Help in finding alternative accommodation
  • Monitoring of health and well being.
