What is a support plan

 

Support Plan Assesment

 

 

After the initial assessment our staff will agree a Support Plan with you. This will include:  

  • Setting up a visiting schedule where a Care Co-ordinator will visit you on an agreed visiting pattern that meets you needs (e.g. once per week, or once per month on a set day)
  • Assisting you to make a claim for any welfare benefit which you may be entitled to, for example, Attendance Allowance, Disability Allowance, Income Support, or Housing/Council Tax Benefit.
  • Liaising with Health and Social Care Agencies (or Voluntary Organisations) regarding the provision of care and support services in your home.
  • Record any services that your Care Co-ordinator may have recommended, for example:
  • Aids and Aaptations.
  • Grab Rails.
  • Occupational Therapy referral for bathing assessment.
